The following information is provided as a general outline of the benefit plans. Detailed explanations and the plan documents will prevail.
Health Plan
Atlantic General Hospital/Health System is pleased to offer three medical options; High Option, Standard Option and High Deductible Option. All three medical options utilize the Atlantic General Hospital/Health System network as well as the National BlueCross BlueShield PPO Network. These plans offer participants the option of seeking care in or out of network. Prescription drug coverage is provided under each medical plan through Express Scripts. The prescription drug plan gives participants the option of purchases at retail pharmacies, mail order, and at the hospital’s AGHRx RediScripts Pharmacy, which offers the lowest prescription co-pays.

Dental Plan
Benefits are administered by Guardian Dental PPO. This program allows you
freedom of choice in selecting your dentist. You can improve your benefits
by choosing a participating Guardian PPO dentist.
Our Dental Plan offers diagnostic and preventive services paid at 100%
of UCR. All other covered dental procedures are paid after a low $50 per
person ; $150 per family deductible and coinsurance.

Voluntary Vision Plan
Associates may purchase vision insurance coverage through Vision Service Plan (VSP). VSP offers a large national network of optometrists and ophthalmologists for your care and you have a choice of using In Network or Out of Network providers.

Life Insurance & AD&D
We offer basic life insurance and accidental death & dismemberment insurance at no cost to full time associates. The value is one times your annual salary (maximum benefit $100,000). This benefits is convertible and portable.
Voluntary Life Insurance
Associate Coverage: from a minimum of $10,000 to a maximum of $250,000,
but not more than 5 x your annual earnings, without medical evidence during
eligibility period. This benefit is portable.
Spouse Coverage: From a minimum of $5,000 to a maximum of $25,000 without
medical evidence during eligibility period, not to exceed the same amount
of the associate’s amount of coverage.
Dependent Children: eligible for $10,000 per child

Voluntary Short-Term Disability
Voluntary Short Term Disability provides you weekly benefits payments to help replace lost income should you become disabled due to non-occupational accident or sickness. We offer 2 plans: one has a 1 week wait period and the other has a 2 week wait period. Short term disability has a duration of 13 weeks. You select a plan that pays from $100.00 to $2000.00 per week, not to exceed 60% of your basic weekly salary.
Eligibility: Any associate consistently scheduled 20+ hours weekly.
Long-Term Disability
Eligibility: Full-time associates working 35+hours weekly, at
no cost to the associates.
There is a 90 day waiting period before benefits are payable.
60% of monthly salary is covered to a maximum of $15,000 per month.
Partial disabilities are covered after the waiting period has been satisfied.

Flexible Spending Accounts for Dependent Care/Health Care
If you participate, you can elect to have a specified amount of pretax money deducted from your paycheck each pay period (not to exceed $5,000 for Dependent care; $2,800 for Health Care annually). These dollars are set aside in a flexible spending account. After you submit a receipt for a qualifying dependent care expense, you will be reimbursed from this account. On a Heath Care FSA’s are funded up front by the employer on a credit card. There are certain qualifications and rules defined by the IRS you must follow to participate.
Eligibility: Any associate consistently scheduled 20+ hours weekly.
Tax Sheltered Annuity (TSA)
A 403b TSA is a unique long-term savings and investment program that offers you the following: You choose the amount you want to save; your contributions are automatically payroll deducted; and you will not pay any current federal income tax on your TSA savings. Employer matching contributions after one year of eligibility, to be determined annually by the Board of Director. Our dedicated financial advisor will provide detailed information about contributions and investment options. We match .50 on the dollar up to and including 5%. We also offer a Roth 403b post tax voluntary option.

529 Plan College Fund
A 529 Plan is a tax-advantaged program which can help your college savings grow faster than other taxable savings options. Our dedicated financial representative will provide detailed information about contributions and investment options.

Vacation, Holidays, Personal Leave, Sick Leave (Paid Time Off) Plan
Paid Time Off provides associates with continuation of pay during absences
from work. All regular full-time and part-time associates are eligible
to participate. You can accrue:
Full Time Hourly Employees:
23 days per year with maximum accrual of two years (368 hours) for the
first five years.
28 days per year with maximum accrual of two years (448 hours) after five years.
Salaried Employees:
28 days per year with maximum accrual of two years (448 hours) for the
first five years.
31 days after five years with maximum accrual of two years (496 hours).

Eligibility: Any associate consistently scheduled 20+ hours weekly
Tuition Assistance - To be eligible, you must be employed full or part time with AGH/AGHS
for three months. Tuition and fees are reimbursed at 100% of expenses
up to the maximum allowable amount per fiscal year for completion of courses
in which a “C” or better grade is received. Associates that
access the maximum allowable amount in the most recent twelve-month period
are required to commit to one year of service with AGH/AGHS. If the reimbursement
is less than the maximum, the time commitment will be pro-rated.
Bereavement Pay - up to three scheduled days (24 hours) for death of immediate family member.
Jury Duty - paid for any scheduled hours less any monies paid by the court.
