A Few Points to Remember to Ensure Your Success with the Program

1. To qualify for our bariatric surgery program, you must:

• Have a body mass index (BMI) of 35-39 and a health condition such as high blood pressure, diabetes, high cholesterol, sleep apnea, etc., or
• Have a BMI of 40 or greater.

You can find your BMI by using this BMI Calculator. Or, you can ask your doctor.

2. You must also meet and fulfill all insurance requirements to be a candidate for surgery. If you have questions about whether you need to bring medical records or if you meet the criteria for bariatric surgery after watching the webinar, please ask our staff when you call for your appointment.

We encourage all potential bariatric patients to have a primary care provider who helps you manage your overall health. Ideally, every new patient will come to his or her consultation visit prepared with a referral and office note, if possible, from his or her primary care provider or a specialist.

Any patient with a history of previous bariatric surgery should come prepared with documentation of any procedures. If you have questions about whether you need to bring medical records, please ask our staff when you call for your appointment.
