Diabetes Self-Management Education

Offered by the Diabetes Education Program, this series of four, two hour sessions will address blood glucose monitoring, foot care, nutrition, exercise, and other self-management skills to help manage diabetes.
The program is recognized by the American Diabetes Association (ADA) for quality education, and program staff includes a Registered Nurse, and Registered Dietitian, both of whom are Certified Diabetes Educators.
A referral from your Primary Care Provider is required.
An individual appointment with the nurse, to review your medical history,
will be scheduled prior to the start of the program.
Registration in advance is required. Diabetes self-management is a Medicare
benefit and the cost of the classes is covered by many insurances.
The classes will be on September 9th, 16th, 23rd and 30th from 9:30AM to 11:30AM at theAtlantic Health Center, 9714 Healthway Drive.
